I've added a few more features to the (ruby script) midi2sco app that I wrote a while back (http://www.csounds.com/node/87), that I hope may make it a little more versatile.
It now (by default) separates instrument parameters in the file it generates with tabs rather than spaces. This doesn't hurt the output as a Score file at all, but makes it also acceptable to a spreadsheet program, so you can conveniently tweak parameters further if you wish.
It will also tag each instrument event with the note name, MIDI note number, and velocity, as a comment, so you can find your way around the file more easily.
There is now a way of adding (fixed) elements to the output for an instrument, which can also be used for identifying comments (where an instrument might have more than one source channel for instance), or to supply extra fixed parameters, rather than having to edit them in afterward.

Yet one more update
I've tweaked midi2sco a bit more (2 Feb 2012) and posted the revised archives in the same places.
The new version accepts Sustain/Damper pedal events to extend notes as needed, and can also annotate the score file with Program and Controller events it encounters, giving their occurrence times. (I decided not to investigate adding other controllers like "expression" because i'm not sure they'd be much use. Expression is probably mostly used during a note for example.)
For fun, and because I found it useful for quick checking of generated score files, I've included "play.orc", which simply plays all 15 note channels as sines, and channel 10 as a "brush" or something... It can actually sound quite pleasant... Sort of a quiet Hammond...
